What is Rhinoplasty?

It is a cosmetic surgery conducted on the nose to reshape or resize the nose, and also to correct the breathing problems. This procedure helps to make the nose appear better, and improves any functional disorders. It is a safe method to alter the nose size in relation to the rest of the face. It also can change the width of the nose bridge and reduces the appearance of humps or depressions on nose. In rhinoplasty surgery, management of the bony vault and lateral walls is most often performed with mechanical instruments: saws, chisels, osteotomes, and rasps.Over the years, these instruments have been refined to minimize damage to the surrounding soft tissues and to maximize precision. Power-assisted rasps, burrs, and saws were designed, however, there were limitations such as the expense, increased operative time, risk of soft tissue injury, more extensive exposure, and difficulty performing lateral osteotomies. Recently, a technology that uses sound waves has been developed to contour nasal bones in a precise and far less traumatic way when compared to traditional techniques. Ultrasonic, or Piezo, rhinoplasty is now the most advanced technique available for minimally traumatic nasal bone reshaping and hump removal during rhinoplasty.

The first application of piezoelectric instrument in rhinoplasty surgery was reported by Robionyin 2004. The initial publication reported the use of a piezo scalpel for performing lateral osteotomies through a percutaneous approach. Several months later, Robiony published report on additional applications in rhinoplasty surgery including management of the bony vault as well as medial and lateral osteotomies, and hump removal In 2013 Cochran and Roostaeianreported intranasal lateral osteotomies. What is the procedure?

Rhinoplasty is done under general anesthesia during which the plastic surgeon will add or remove the bones and cartilage from the nose. This is done to change the form or functionality of the nose. The other options to enhance the aesthetics are adding a cartilage graft or remodeling of the tip to make it look younger and fitter. Fillers or fat grafts can also be put in to increase the volume and give that youthful look. Some patients prefer silicone, but it comes with its risks of infection.

Mechanism of Action of Piezotome Device

This Piezotomedevice uses ultrasonic i.e. high speed sound waves to create vibrations in tissue. These vibrations cause gentle breaking apart of tissue surfaces to cut tissues, all while avoiding damage to underlying structures. These devices minimize soft tissue injury, because a frequency of 25 to 29 kHz is utilized to cut bone, although a frequency greater than 50 kHz is necessary to cut neurovascular structures. Piezoelectric inserts have the ability to selectively act on bones and/or hard cartilage, without injuring soft tissues: skin, mucosa, and flimsy cartilages such as the upper lateral cartilages (ULCs) and lower lateral cartilages.

Importantly, the fracture lines created by piezoelectric instruments are very accurate and eliminate the risk of radiating fracture lines encountered with traditional instrumentation. In other words, it treats the areas that need treatment in a highly targeted and precise way, all while keeping damage to a minimum.

What is the Recovery Time?

Recovery after Traditional Rhinoplasty

After the operation a splint will be placed on your nose. The nasal orifices will be covered and packed with paraffin gauze for 24 to 48 hours. You will be required to wear a patch on the upper lip that you will have to change many times a day. In some cases, patients experience any slight bleeding, for which they can use a wet cotton swab and place it on the nose till the bleeding stops.

In 7-9 days the splint will be removed and you will be able to do your day-to-day activities. The bruises & swelling will diminish in the next few days and the nose will get softer over a few months and get back to normal. Avoid sun in the initial days of surgery.

Within 15 days, you can start practicing light sports, but it is recommended to resume heavy exercises only after a month and consulting your surgeon.

Recovery after Ultrasonic Rhinoplasty

The recovery afterultrasonic rhinoplastyis similar to traditional rhinoplasty. You still need to wear a nasal splint for seven days. You still need to sleep with your head of bed elevated. But since there is far less intra-operative trauma, the patient experiences less swelling, and almost no bruising.

The major difference is that your nose looks more presentable to the public early on and the long-term final result is enhanced once full healing has occurred.

Are there any side effects?

Rhinoplasty is a very safe procedure and when performed by an experienced board-certified plastic surgeon, has minimal to no side effects. Few of the common side effects may include swelling, bruising, infection or skin rash that is temporary and will diminish within next few days. During the first weeks after surgery, breathing may be difficult due to incisions, scars, mucous hypertrophy and valvular, collapse. This will disappear in a few weeks.
