How EVL is Performed

In expansion vibration lipofilling, 4- to 5-mm angled basket cannulas are used and attached to a power-assisted liposuction unit. Power-assisted tunnelization in a multi-directional and multilayered fashion is used to prepare a matrix at the recipient site. This ensures maximal space for fat grafting and limits the size of the injected fat lobules. 

The tubing attached to the cannula is used as a conduit to propel fat out of the tip of the cannula. Fat flow is induced by the use of a peristaltic pump from a canister filled with fat. In this manner, simultaneous filling, expansion, and shaping could be performed uniformly.

Why EVL is Such a Safe Procedure

Expansion vibration lipofilling allows for recipient-site expansion by using basket cannulas, creating a wider potential space and ability to separate and equalise tissue. The larger diameter of the cannula makes its more significant, blunter tip less likely to perforate a venous structure. The fixed-angle cannula can be rotated to increase cannula excursion and efficiency using fewer passes, and also allows it to be oriented superficially to minimise deeper tissue penetration.  

Concurrently, fat is deposited into the expanded area with the basket cannula, effectively serving as a stent followed by the immediate deposition of the graft to occupy the stented space. Also, the use of vibration (oscillations) allows for the homogeneous distribution of fat lobules. More thorough and dispersed fat translates to more surface area contact between grafted fat lobules and recipient tissue leading to greater fat survival.

Brazilian Butt Lift Procedure

Brazilian butt lift is a specialised fat transfer procedure that augments the size and shape of the buttocks without implants. Excess fat is removed with liposuction, and a portion of this fat is then strategically injected into the buttocks. A large volume of donor fat is needed to achieve a pleasing gluteal shape. 

Depending on the body size and gluteal dimensions of the patient, injection volumes range from 450 to 1100 mL or more per side. Traditional techniques for injection of such large volumes of fat require long operating times and are associated with surgeon fatigue and morbidities in the patient.
